Through this project, we will seek to understand the challenges and barriers of designing alternative payment models that are more supportive of population health, pursue activities to help overcome those barriers, and identify the conditions and facilitators that can underpin those community-focused payment models. Employing an emergent strategy, AcademyHealth will continuously reflect back on each step of the project, which will allow us to build on and redirect, as necessary, our efforts and lessons learned over the course of the project. This project will bring together experience and knowledge from across AcademyHealth’s range of expertise in delivery and payment system reform, health services and policy research, public and population health, health information technology and exchange, and quality improvement.
