This webinar featured the latest evidence and experience on improving care for complex patients from experts in the United States and around the world. Health care researchers, policy makers, and other stakeholders are increasingly looking for innovative ways to transform the care provided to complex patients in order to better coordinate across settings. This webinar explored the current landscape of caring for complex patients, highlight some emerging innovations to improve care, and discuss the challenges that still remain.
