Now that the State Quality Improvement Institute is in full swing, with the Kick-off Meeting successfully behind us, we are working on designing the technical assistance offerings to participating states.

Cyber Seminars

Please mark your calendars for the following times, as they are the dates that have been set aside for the 4 Cyber Seminars. Each Cyber Seminar will feature faculty, some of whom were at the Kick-off Meeting, along with potentially new faculty, based on each state's final action plan needs. Each Cyber Seminar will cover a distinct topic.

Primary Care Innovation: The Patient-Centered Medical Home – A Commonwealth Fund/Alliance for Health Reform Briefing E-Forum
October 24, 2008 - On September 22, the Alliance for Health Reform and The Commonwealth Fund sponsored a briefing to address the need for better care coordination, better insurance coverage for chronic care, and new incentives to develop medical homes. Panelists were: Robert Berenson, Urban Institute; Diane Rittenhouse, University of California San Francisco ; and Duane Davis, Geisinger Health System. State Roles in Multi-Payer Medical Home Pilots: A free Web Seminar presented by the National Academy for State Health Policy (NASHP) and the Patient Centered Primary Care Collaborative (PCPCC), with the support of The Commonwealth Fund
This is the third in a series of web seminars on the patient centered medical home concept and the value it brings to primary care. This program will focus on existing multi-payer medical home pilot programs, covering defining characteristics of each, reimbursement methodologies, how the pilots are supporting practice transformation, and the state role in the effort.
