On-site Registration
You may register on-site as availability permits. Visit this web page after Monday, January 23, to confirm that on-site registration will be available.
Individual Member Rate
AcademyHealth individual members receive a reduced conference registration rate. Membership must be current at the time of registration. If your membership has expired and you do not renew at the time of registration, you will be registered at the non-member rate and receive a balance due notification. If you join AcademyHealth while registering, you are eligible to receive the reduced individual member rate. For membership questions, call 202.292.6700.
Confirmations
Registration confirmations will be sent to the e-mail address provided on the registration form. If you do not have an e-mail address, the notice will be faxed to you. Please call 202.292.6738 if you do not receive confirmation within two weeks after registering.
Cancellations
All cancellations must be made in writing and received before Friday, December 30, in order to receive a full refund, less a $25 processing fee.
Modifications/Substitutions
All substitutions and adjustments will be assessed a $25 processing fee. Substitutions will be accepted only once per registrant and must include written notification from the original registrant and a completed registration form for the substitute participant. If a non-member substitutes for a member, the non-member rate applies.

Hotel Reservations
The conference will be held at the Renaissance Washington, D.C. Hotel, 999 Ninth Street N.W., Washington, D.C. 20001. A block of rooms has been reserved at the hotel for $199 single/double, per night. All reservations must be made by Friday, December 30, 2005, after which reservations are accepted on a space and rate availability basis only. All reservations must be guaranteed with first night's room and tax by cash or credit card. The deposit will be applied to any guaranteed no-show reservations. Please make your reservation directly with the hotel at 202.898.9000 or with Renaissance Worldwide Reservations at 1.800.468.357 and indicate that you are attending the AcademyHealth National Health Policy Conference.
