Overview 

This webinar provides a brief overview of AcademyHealth’s 2017 Concordium Meeting and features the work of two awardees, who present on their innovative electronic health data projects focusing on health, housing, and school-based health.

The Concordium Meeting convened a small group of experts on November 17, 2017 to discuss strategic development and use of evidence to transform health systems, with an emphasis on data sharing and collaboration. The Meeting included awards for two public health practitioners working across sectors to improve population health through enhanced data exchange.

During this webinar, Dr. Margo Edmunds discusses key take-aways from the Concordium Meeting on topics including cognitive computing, analytics and data science, and use of data to promote health equity. Dr. Sujani Kakumanu discusses her work on a health information exchange and school based partnership with health providers focused on addressing the needs of children with asthma. And Dr. Alastair Matheson discusses his work on a cross-sectoral collaboration focused on data exchange and coordination between the health department and the housing authority.

At the conclusion of the session, participants will:

  • Be familiar with AcademyHealth’s recent work and convening on health system transformation, collaboration, and data sharing, and
  • Have an understanding of two innovative data sharing projects focused on cross-sectoral collaborations.
